Sam Breyfogle, LPC, LMFT
Sam Breyfogle is a Licensed Professional Counselor and Licensed Marriage and Family Therapist serving patients at Alleviant Integrated Mental Health in Rogers, Arkansas. She holds a master’s degree in Mental Health Counseling and Marital and Family Therapy from John Brown University, earned in 2022. Sam focuses on individual therapy and brings a rich background in social work, nonprofit leadership, and inpatient behavioral health.
Before joining Alleviant in June 2025, Sam worked in a variety of mental health and community-focused roles including child welfare with DHS, crisis support at Northwest Arkansas Women’s Shelter, and therapy roles at Piney Ridge Treatment Center, Mercy, and Vantage Point Behavioral Health. Her unique journey includes founding an upscale thrift boutique that funded local nonprofits.
Sam’s passion for therapy was born out of a desire to meet people where they are and help them get to where they want to be. She values access to quality mental health care without long waitlists and believes that everyone deserves personalized, goal-focused support.

Patients describe Sam as authentic, positive, and deeply curious. Raised in a military family, she spent her early years in Florida, Hawaii, and California before settling in San Diego. In 2011, she moved to Arkansas where she now lives close to her two adult children and grandchildren. Her mother also lives with her and is an active part of their family life.
Outside of work, Sam enjoys crafting, reading, and spending time with loved ones. A fun fact: in her 20s, she sold Harley-Davidson motorcycles in San Diego — a unique chapter in a life full of meaningful experiences.
